What’s actually happening with gum disease

Gum disease is an infection of the tissue and bone that hold teeth in place. It starts as gingivitis — inflammation caused by bacteria in plaque at and below the gumline. At this stage, with professional treatment and improved home care, it’s fully reversible.

Left untreated, gingivitis progresses to periodontitis: the infection spreads to the supporting bone. Bone loss at this stage isn’t reversible, but the disease is controllable. With proper treatment and ongoing maintenance, periodontitis doesn’t have to progress, and most patients keep their teeth.

Nearly half of adults over 30 have some form of gum disease. It’s not a reflection of how much you care about your health — it’s often asymptomatic and goes unnoticed until a dentist catches it during a routine dental exam and cleaning.

Common questions

The most reliable way is an exam. Gum disease is often asymptomatic in early stages — many patients have no pain or obvious signs until it’s progressed. Common early signals include bleeding when brushing or flossing, persistent bad breath, and red or swollen gums. If you’ve noticed any of these, it’s worth having your gum health evaluated.

It depends on the stage. Gingivitis — the early form — is fully reversible with professional treatment and consistent home care. Periodontitis (more advanced disease involving bone loss) is not reversible, but it’s manageable. With the right treatment and maintenance schedule, it can be controlled and kept from progressing.

Scaling and root planing, the most common treatment for periodontitis, is done with local anesthesia. You’ll feel pressure but not pain during the procedure. Afterward, some tenderness and sensitivity is normal for a few days. Most patients find it easier than they expected.

Untreated gum disease progresses. Pockets deepen, bone loss increases, and eventually teeth can loosen and require extraction. The condition also doesn’t resolve on its own — bacteria below the gumline need to be removed professionally. Treating it early keeps options open and costs lower.
